Please complete the form below to obtain an official copy of your Mother McAuley transcript or medical forms.  You may also download and print a PDF of the form to drop off in the Main Office or mail to:

There is a cost of $5 per transcript you wish to send, and $2 per medical record.

For transcripts for one of our dual/college credit program courses, please contact the college/university registrar's office.
